Write down things that may be concerning you so that you don’t forget.

Write down a list of questions.

Look up information about your or your child’s concerns (please see our helpful links). This will help you know what to talk about with your Speech Pathologist and/or Orofacial Myologist.

Make sure you read over and fill out the initial intake paperwork thoroughly so you are prepared to ask any questions during your visit.

Use the “ASK ME 3″ questions guideline:

-What is my main concern?

-What do I need to do?

-Why is it important for me to do this?

Ask the questions that you wrote down before your visit.

Ask the therapist to write down any terms you didn’t understand.

Find out contact information in case you think of questions after your visit.

Go over your notes to see if you understand what was said.

Call or email your therapist if you can think of any further questions.

See if you can find out more about what you were told – do a little research on your own.

Ask other people you know who may have the same areas of concern.

Search the internet for more information.

To the office

Written Directions to The Honolulu Office:

• From Kapiolani Blvd (Head northwest)

• At the Stop Light Turn left onto Cooke St.

• Turn left at the 1st cross street onto Kawaiaha‘o St.

• Destination will be on the Left (The Imperial Plaza)

• Take a ticket from the guard shack.

• Park in any green Customer stall on levels 1-3.

• Be sure to get validation at the office (stamp the blank side of the ticket)

Written Directions to The Kapolei Office:

• From Kapolei PKWY

• At the stop sign turn Right onto Kamaaha ave (toward the ocean/Makai)

• Turn Left on Kakala St.

• Continue to Alohikea St.

• Destination will be on the left on the corner of Kakala and Alohikea St.

• Find Street parking on Kakala or Alohikea St.

• Entrance to the office is on Kakala Street
